Include:
Description of the core measure.
How the measure is evaluated and current evaluation results.
Regulatory agency or policy affecting the core measure.
Evidence for intervention/action to improve the plan.
TeamSTEPPS algorithm/outline for core measure improvement.
Include the leader’s role in developing/implementing/executing the plan. Support information in the plan with current scholarly literature. The paper, as well as the reference list, must be formatted per the Publication Manual of the American Psychological Association 6th edition.
