Instructions
This assignment will allow the student to use critical thinking in diagnostic reasoning and clinical judgment as well as consider the life cycle and cultural factors.
For each of the case studies, complete the following:
Identify the subjective and objective data, and at least one nursing diagnosis.
Include wellness, health promotion, and disease prevention activities.
Also discuss developmental, socioeconomic, and cross-cultural considerations for each situation.
In addition:
Relate expected (normal) findings to those described
Discuss any additional information that might be needed before a diagnosis can be made
Identify at least one relevant nursing diagnosis for any actual or potential problem identified.
Case Studies
During an examination, the nurse notes that a female patient’s nipple is covered with a small, crusty patch that extends almost to the areola. She tells the nurse that her nipple has felt “itchy, with some burning” for a few weeks but she thought her bra was irritating the area. What may be the problem and how will it be diagnosed?
C. W., a 15-year-old male high school student, has come to the family doctor for a school health physical. During the examination, breast enlargement is noted, and a smooth, firm, movable disc is felt in the area of the breast. What do these findings indicate?
